Anoka Royal Ambassador Candidate Agreement and Release

In consideration of being allowed to submit my application to be considered for selection as an ANOKA ROYAL AMBASSADOR, I, _______ do hereby state as follows:

(1) I have voluntarily entered the ANOKA AMBASSADOR PROGRAM at my own risk.

(2) My age is __ and I will be a senior at Anoka High School or a graduate of Anoka High School, or a five-year resident of the Anoka High School attendance area and have attended a private school.

(3) I have read and signed the Anoka Ambassador Registration form and have entered said program subject to all rules and regulations thereof.

(4) I am single. I have never been married, nor have I had a marriage dissolved or annulled. I have never had a child, I am not now pregnant, nor have I ever been pregnant. I promise and agree that I will not become engaged to be married, I will not get married, nor will I cohabitate with an unrelated single male during the term of my reign, if I am selected to serve as an ANOKA ROYAL AMBASSADOR.

(5) I hereby authorize and permit Anoka Halloween, Inc., its officers, agents, and employees, to use my name and picture in such a manner as they see fit during the period of the pageant and thereafter, provided that my name and picture shall be at all times used for publicity purposes. Any pictures taken for the program and paid for by the Ambassador’s committee shall become sole property of the Ambassador’s Committee and cannot be reproduced or distributed without the expressed written consent of the Ambassador’s Committee.

(6) If I am selected to serve as an ANOKA ROYAL AMBASSADOR, I shall relinquish all other titles that I now hold and in consideration therefore, agree to be known as an ANOKA ROYAL AMBASSADOR. I will fully cooperate with Anoka Halloween Inc., and its officers in all of its functions and public relations programs and I hereby agree that any pictures of me and other publicity material arranged for or produced as Ambassador-elect may be used by Anoka Halloween, Inc., its officers and agents as they see fit without any claims thereon by me of any kind or nature whatsoever.

(7) It is understood and agreed that if selected an ANOKA ROYAL AMBASSADOR, I am free to choose
employment, but such employment will be such as will allow me to be free for such public appearances, functions, trips, etc., as shall be assigned by the Ambassador’s Committee. It is also understood that I will not enter any other pageant competitions during the term of my reign as an Anoka Ambassador, and further that if I do enter other competitions, I will first resign my position as Anoka Royalty so that an alternate candidate selected by the judges can be appointed to fill my position.

(8) I understand that the Anoka Royal Ambassador Program only accepts 50 candidates. If multiple
applications are received on the same day, I understand that the candidacy selection process is based upon a lottery system and my application may not be chosen.

I do hereby release, remise, and forever discharge Anoka Halloween, Inc., its officers, agents, and employees, and each of them, and agree to hold said Anoka Halloween Inc., and its officers, agents, and employees harmless from any and all liability direct or indirect, for personal injury or property damage which I may suffer during the Anoka Ambassador Program, and at any subsequent appearances made by me on behalf of said organization.
